How it Works

This activity is based on ‘Paving the Path to Wellness’, which was designed by the Harvard Medical School. The original version is available to download below.

Complete each of the categories below by rating yourself on a scale of (1-5).

(1) Never do this

(2) Only rarely do this

(3) Sometimes do this

(4) Often do this

(5) Do this regularly as part of my routine

Your scores will be accumulated and mapped onto a chart at the end. This will give you a broad sense of your overall wellness.

TROUBLESHOOT: If you experience any difficulty scrolling down, ensure that the cursor is clicked inside the activity boxes. The chart should work with all browsers but Google Chrome may experience difficulty if you have a firewall installed on your computer.
